Job Title: Physical Therapist (PT) SNF / LTCCentra Healthcare Allied is seeking a skilled Physical Therapist to join our team for a travel assignment in a skilled nursing facility (SNF) or long-term care (LTC) setting.Job Summary:We are looking for a highly motivated and experienced Physical Therapist to provide high-quality patient care in a dynamic and fast-paced environment. The ideal candidate will have a strong background in SNF/LTC settings and be able to work effectively with interdisciplinary teams to achieve optimal patient outcomes.Responsibilities:

• Provide physical therapy services to patients in a SNF/LTC setting, including assessment, treatment, and documentation of patient progress.
• Develop and implement individualized treatment plans to meet the unique needs of each patient.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Conduct patient evaluations and assessments to identify areas of improvement and develop treatment plans to address these needs.
• Provide education to patients and their families on physical therapy techniques and strategies to promote independence and optimal function.Requirements:
  • 1 year of recent experience as a Physical Therapist in a SNF/LTC setting.
  • Current Physical Therapist license in the state of practice.Benefits:
