Virtual Care Coordinator

Job Description:

A company is looking for a Virtual Care Coordinator.
Key Responsibilities

Conduct remote patient check-ins and prepare for clinician visits
Document medical information in the EMR and assist clinicians during virtual visits
Coordinate care by managing referrals, prior authorizations, and medication refill requests

Required Qualifications

High school diploma or equivalent
2+ years of experience in a virtual care or healthcare support role
Familiarity with virtual care workflows in a primary care or chronic care setting
Comfort with remote tools and EMR platforms, preferably Athena
Knowledge of obesity medicine or chronic care workflows is preferred
